After we have this information, we will send you a link to establish a patient portal where you can complete the necessary documents and upload your insurance information. You can provide us with this information via voicemail, or text message.

Step 2: Complete the documents in the portal. This includes information about your background to help us get you scheduled with the right person. It also allows us to obtain the information needed for insurance coverage purposes.

Step 3: After your documents are completed, we will call, email, or text you (depending upon your preference) to get you scheduled! Please allow us a few days to get you connected to the right person.
